Class of 1958 Questionnaire

Instructions for Class of 1958 Memory Book Questionnaire


Please take a few moments to respond to the Class of 1958 Memory Book questionnaire, even if you don’t plan to join us at the reunion. Everyone (or couple) who submits a questionnaire will receive a copy of the memory book at no charge. Even if you only attended Willamette for one, two or three years—you are still a member of the Class of 1958. That’s what a reunion is all about!
  • If you have a spouse who is also from the Class of 1958, please submit a separate questionnaire form.
  • Since your writing will not be edited, answer as you would like it to be read.
  • Each person is allotted one page in the Memory Book, single-spaced; font size 12, so no resumes please, just your own words telling your own story.
  • Send us a recent photo or snapshot. You can send your photo electronically to alumni@willamette.edu, or by mail to Alumni Office, Willamette University, 900 State Street, Salem, OR 97301. Be sure to attach your name and address to each photo submitted.
  • If you have an old snapshot from our four years at Willamette send it to the address above. We will be creating a collage of candid shots at the end of the memory book. Again, be sure to attach your name and address to each photo.


    • Extended deadline for submitting Memory Book Questionnaires is April 20, 2008. Don't delay!
